Insurance

In-network

We are in-network with Cigna, United Healthcare, Blue Shield (except ASH**), and worker's comp (MedRisk, Sedgwick and CorVel).   We are happy to verify your insurance benefits.  Please fill out the forms below and we will get back to you within 1-2 business days.  We will submit claims for in-network patients at no cost.

Out-of-network

We accept some out-of-network insurance.   We can verify your insurance benefits and get back to you within 1-2 business days.  Providers do not guarantee payment from insurance.  Patient is responsible for the treatment cost and payment is due upon treatments provided.


**We do not accept American Specialty Health (ASH) and therefore, we do not fill out MNR form for the claims.  We will provide superbill upon request.   There is a $15 document processing fee for each MNR form request (up to 5 visits) to be filled out.  Provider do not guarantee payment from ASH.  

Other insurance:

Starting in Jul 2023, We no long accept new patients for Optum Health.  

We do not accept ASH, Kaiser, MediCal, Medicare, VA or Triwest.

Reimbursement for out-of-network pateints

Tips to get your medical reimbursements:


  • If you have out-of-network benefits, you can submit the claim along with the 'Superbill' for reimbursement.  Superbill can be downloaded from our patient portal or we can provide them upon request.  
  • Utilize your FSA/HSA contributions to maximize  your tax savings.  Please note that herbal supplement cannot be expensed with FSA.
  • After you have met your deductibles, if applicable, your insurance will cover full or partial of your treatment cost.  Please check with your insurance for the allowed number of visits per year and other restrictions that may be applied  (e.g. maximum limit in dollar value per year).
  • If you have reached the maximum out-of-pocket for the plan year, all copayments or co-insurance will no longer applied.
